A Case of Bilateral Tuberculosis Otitis Media Following Nasopharyngeal Tuberculosis

Abstract
Bilateral tuberculous otitis media (TBOM) and nasopharyngeal tuberculosis (NPTB) are rare forms of TB. To date, no case of NPTB leading to bilateral TBOM has been reported. This paper presents the unique case of a 42-year-old female in whom NPTB lesions progressed to bilateral TBOM within a month of practicing inappropriate nasal irrigation. In addition to the history of improper technique, the comparison of imaging findings before and one month after nasal irrigation led to the conclusion that incorrect nasal irrigation significantly contributed to the rapid spread of NPTB to both middle ears. Based on these findings, it is suggested that even before diagnosing NPTB, it is essential to educate patients on the correct nasal irrigation method. In particular, if NPTB is diagnosed, avoiding nasal irrigation may reduce the risk of NPTB spreading to the middle ear.
